CASA Playhouse 2009 Disclaimer

The official winner of the CASA Playhouse is subject to the following disclaimers:

 The winner of the CASA of McHenry County CASA Playhouse raffle will be drawn and announced on Thursday, August 6 at the McHenry County Fair. Winner need not be present to win however the winner must claim the CASA Playhouse within 30 days of notification of being named the winner.

Winner of the CASA Playhouse must sign a release of liability form and IRS Form W-9 prior to collecting the CASA Playhouse prize. Failure to sign this release will result in a forfeiture of the CASA Playhouse with no additional prizes or compensation given in place of the CASA Playhouse.

CASA of McHenry County will only provide curbside delivery of the CASA Playhouse to the location of the winner’s choosing unless other arrangements have been made between CASA of McHenry County and the winning party. CASA of McHenry County will provide delivery of the CASA Playhouse to locations within McHenry County only. Delivery of the CASA Playhouse to locations outside of McHenry County may be subject to a delivery fee.

Winners maybe subject to taxes associated with the winning of the CASA Playhouse.

Board members and staff persons of CASA of McHenry County and their immediate family members are excluded from winning the CASA Playhouse.
